A health record which includes medical history, physical examinations, screening results and emergency information is kept on each student throughout his/her school career. Please forward all medical updates such as; immunizations, medications, physical exams and changes in health status as soon as possible. Please be sure to update emergency contact phone numbers including cell phones as they change.
Physical exams are required for all students entering preschool, kindergarten and Grades 7 and 11. In addition all students entering Stoughton Public Schools from outside Massachusetts must present a physical exam done less than one year ago prior to entrance.
